·         Apollo unveils its annual report – Health of the Nation 2023 – highlighting the prevalence and growth of NCDs in India

·         Finds an increase in prevalence of early risk factors like Obesity and Dyslipidemia by 50% and 18% respectively between 2019 and 2022

·         ‘MIND HEALTH’ and Sleep emerge as a major unrecognized contributor to NCDs

·         Apollo also enhances ProHealth, a 100% doctor curated and AI-backed health program that captures one’s health status, personalizes tests, predicts risks and guides on a path to wellness, offering a one-stop preventive health solution, ending the era of generic health checks

Apollo, the world’s largest vertically integrated healthcare provider, has released its annual Health of the Nation report that deep dives into the prevalence and growth of NCDs and highlights the importance of the right preventive healthcare measures to ensure India stays healthy.

Dr Prathap Reddy, Chairman, Apollo Hospitals Group, stated that “Preventive healthcare needs to become a national priority. Over the past 3 decades, non-communicable diseases have become the leading cause of death and suffering, contributing to 65% of deaths in India. NCDs affect not just health, but also productivity and economic growth. The estimated economic burden on India is expected be about $4.8 trillion by 2030. As the youngest and fastest growing major economy in the world, the health of our nation is a critical indicator of our future, and the health of our people will determine how effectively we live up to our full potential. What we need is a proactive and highly defined strategy to minimize the impact of NCDs. And the best solution lies in prevention.”

The increasing trend for NCDs and a significant rise in early risk factors

The rise in preventive screenings has also led to an increase in the diagnosis of early risk factors like Obesity and Dyslipidemia (cholesterol irregularities) among Indians across age groups. These are indication of the potential onset of chronic conditions like diabetes and hypertension and a wake-up call for individuals to make early shifts in their lifestyle behaviors.

·         There is a 50% increase in the prevalence of Obesity among Indians between 2019 and 2022.

o   Obesity has seen an uptick in diagnosis in those less than 45 years of age by 43% and by 60% in people over the age of 45.

·         Dyslipidemia or Cholesterol irregularities has seen an 18% increase in prevalence among Indians between 2019 and 2022

o   This is also fueled by a significant increase in its prevalence among those over 45 years of age by more than 35%

Along with these early risk factors, we also see a rise in the prevalence of conditions like Diabetes and Hypertension.

·         Diabetes and Hypertension diagnosis have seen an 8% and 11% increase respectively between 2019-22

o    There is also an increasing risk of Hypertension amongst Indians over 45 years, its prevalence in diagnosis having increased from 14% to 16% in the last 3 years.

o    Chronic stress and anxiety increase the risk of incidence of hypertension by 1.5X and diabetes by up to 2X. Men with chronic stress have double the risk of incidence of diabetes than women.

We administered the PHQ9 Depression Questionnaire for about 2000 people with Type 2 Diabetes at Apollo Hospitals to understand their Mind Health status. Our analysis showed that with every one unit increase in age and BMI, PHQ9 the depression score also increases. About 50% had some form depression, and Diabetes along with any weight related comorbidity is an indicator to screen for depression.

·         The report also puts the spotlight on the importance of getting more frequent or extended health tests for conditions that are prevalent in our families

o    The report finds that Diabetes is prevalent in 1 in 3 people with a history of the condition in their family.

·         Healthy sleep is an important indicator of overall health.

o   In one study with 20,000 people, 47% have sleep problems and 52% have impairments in their mind health state, and 1 in 3 have both issues.

o   2 out of 3 individuals do not have an optimal gap between dinner and bedtime, which is critical for good quality sleep. It is either less than 1 hour or more than 2 hours, both scenarios leading to sub optimal sleep quality.

·         Impaired metabolism is also closely linked with digestive irregularities, such as gas/bloating, heaviness post-meal, burning sensation, belch/burp. The analysis found that 64% of people had digestive irregularities, going up to 81% amongst diabetics.

Many ‘Indias’ in one India

Our diversity in lifestyle has led to the emergence of different NCD trends across regions, likely impacted by our regional diet preferences.

·         While liver diseases saw maximum prevalence in East (at 50%), its lowest impact relatively is in the South (28%)

·         West has seen the least relatively the lowest prevalence of diabetes (15%) while South has the highest (27%)

·         Obesity trends have been similar across regions, ranging between 22-24%

·         Dyslipidemia (cholesterol) has the highest prevalence across all regions, with North seeing the most (48%), followed by West (41%), East (39%) and then South (37%).

Power of prevention

The silent epidemic of NCDs is behind 40% of all hospital stays. While there is an increase in Indians taking preventive screenings, these are still largely symptom led. Regular and comprehensive health checks, starting at a young age, can significantly reduce mortality rates, disease incidence as well as the frequency of hospital visits through our lifespan.

There is also a need to reimagine preventive health assessments and design them to be more suitable to the Indian population.

For instance:

·         The median age of breast cancer diagnosis from mammograms done at Apollo in the last year was 54-57 years; lower than that seen in mature markets (where the median age of detection is 62-65 years). This suggests an earlier onset of disease in Indian women, warranting annual screening for women from 40 years onwards.

o   According to clinically published sources, early diagnosis through preventive checks of breast cancer can reduce mortality rates by up to 30%.

·         Undetected sleep disorders often lead to chronic health conditions due to late diagnosis and oversight by the healthcare systems

o   Obstructive Sleep Apnea (OSA) is known to increase the risk of heart problems, and is often screened only in the obese (BMI > 30) and with neck circumference > 40cm

o   Apollo’s study of people with Type 2 suggests that intermediate to high risk of OSA occurs even at a lower BMI (26) in the population screened, with a majority (91%) having a neck circumference < 40cm, thus highlighting the need to screen for OSA rigorously, irrespective of the BMI, especially for high-risk cohorts like patients with Diabetes.

Enhanced ‘ProHealth’ by Apollo

In a bid to strengthen the preventive healthcare ecosystem, Apollo has launched an enhanced ProHealth, India’s most individualized preventive health program that brings together the power of AI to help individuals stay ahead of potential NCDs. Combined with Apollo’s pioneering experience of 40 years, the healthcare group has developed new technologies based on AI and ML for predicting risk scores and developing structured lifestyle programs with new models of care that lead to improved clinical outcomes.

What differentiates ProHealth:

1.       ProHealth is a 100% doctor designed health program

2.       It creates a first-of-its-kind health profile that determines the areas of concerns and custom designs a unique health check for every individual

3.       It integrates predictive health metrics through a sophisticated AI engine to determine potential areas of prevention

4.       In addition to the results, every individual also has access to health mentors to transform their lifestyle and make the program truly preventive in every sense

Technology is transforming the approach for dealing with health risks and enabling early detection, and therefore must be integrated in preventive screenings. With a firm belief that AI and big data will play a key role in predicting risks and delivering personalized health solutions, Apollo’s ProHealth today is India’s most individualized preventive health program. Apollo also strongly believes that a laundry list of tests is not an effective solution to get to the root of the problem. For instance, there is a clear data-driven correlation between diabetes-obstructive sleep apnea-heart problems OR weight related comorbidities & depression, however generic health checks are unable to deduce these. The need of the hour is true personalization and prediction, which forms the foundation of Apollo ProHealth. Additionally, by offering convenient access to health mentors as a part of ProHealth, every individual is empowered to take charge of their own health and create a long-term and permanent impact in the battle over NCDs.

Apollo ProHealth is backed by a predictive algorithm that captures one’s health status, predicts potential health risks and individualizes every health check. A personal health risk assessment is digitally available to every individual, post which they can visit the nearest Apollo facility to them to conduct the set of customized tests recommended for them.

Cancer is a global health issue and among the leading causes of death worldwide. According to the World Health Organization, cancer leads to nearly one in six deaths worldwide. It is a term used for a large group of diseases with a defining feature of abnormal cell growth, usually beyond their boundaries and expected numbers. As per interactive session and inputs received from Dr Avinash Gupta, a senior Physician practicing in New Jersey, USA, with modern advancements in technology, the treatment of different kinds of cancer has come a long way. However, the most effective strategy for managing the global burden of cancer is effective prevention and early screening of cancers.Early detection of cancers usually leads to a better prognosis for the patient. Late stage metastatic cancer, or cancer that’s spread to other parts of the body, is much harder to treat. People with a diagnosis of stage 4 cancer have a 5-year relative survival rate.In comparison, those with stage 1 cancer have a 5-year relative survival rate of about 90 percent. There are a number of tests available today that can help detect early signs of cancer or even a predisposition to developing it. Recently With Government of India support AlIMS Delhi  has set up  National Cancer Institute, Jhajjar Haryana and  Chief NCI reiterates in Public lectures that we are able to save Cancer patients if they come for consultation at early stage but India due to less awareness, no female disclose her small breast lump either to her husband or her father and moves to hospital at 4th/Last stage of cancer .Inadequate awareness and unavailability of screening modalities in developing countries lead to patients presenting with cancers at a later stage, which are difficult to treat and life-threatening. According to the American Cancer Society, healthy women/men should undergo periodic screening for breast, cervical, colon, prostate and other cancers.

Early signs of cancer

One of the best ways to fight the disease is to catch it in the early stages, when it’s more treatable. The problem is that the warning signs for many kinds of cancer can seem pretty mild.

Take a look at these signs and symptoms. Some are linked more strongly to cancer than others, but all are worth knowing about — and even talking over with your doctor.

1. Breast Changes

Symptoms of breast cancer include a lump in the breast, bloody discharge from the nipple and changes in the shape or texture of the nipple or breast. A cancer that forms in the cells of the breasts. Breast cancer can occur in women and rarely in men, However as per health journal reports, in 2017 alone, 2,470 men diagnosed with breast cancer.  “Men tend to ignore breast lumps because breast cancer isn’t on their radar,” Meyers says, warning that in men those cancers are also, “diagnosed much later.” Don’t take any chances.

2. Problems during urination

Many men/women have some problems peeing as they get older, like:

  1. A need to pee more often, especially at night
  2. Dribbling, leaking, or an urgent need to go
  3. Trouble starting to pee, or a weak stream
  4. A burning sensation during urination

An enlarged prostate gland usually causes these symptoms, but so can prostate cancer. See your doctor to check on the cause of the problem. They’ll give you an exam to look for an enlarged prostate, and they may talk to you about a blood test (called a PSA test) for prostate cancer.

3. Blood in Urine or Stool

These can be among the first signs of cancer of the bladder, kidneys, or colon. It’s a good idea to see your doctor for any bleeding that’s not normal, even if you don’t have other symptoms, Lepor says. Although you’re more likely to have a problem that’s not cancer, like haemorrhoids or a urinary infection, it’s important to find and treat the cause.

4. Skin Changes

When you notice a change in the size, shape, or colour of a mole or other spot on your skin, see your doctor as soon as you can. Spots that are new or look different are top signs of skin cancer. You’ll need an exam and perhaps a biopsy, which means doctors remove a small piece of tissue for testing.

5. Changes in Lymph Nodes

Tenderness of swelling in your lymph nodes, the small bean-shaped glands found in your neck, armpits, and other places, often signal that something’s going on in your body. Usually, it means your immune system is fighting a sore throat or cold, but certain cancers can also trigger the changes. Have your doctor check any swelling or tenderness that doesn’t get better in 2 to 4 weeks, Meyers says.

6. Trouble Swallowing

Some people have trouble swallowing from time to time. But if your problems don’t go away and you’re also losing weight or vomiting, your doctor may want to check you for throat or stomach cancer. They’ll start with a throat exam and barium X-ray. During a barium test, you drink a special liquid that makes your throat stand out on the X-ray.

7. Heartburn

You can take care of most cases of heartburn with changes to your diet, drinking habits, and stress levels. If that doesn’t help, ask your doctor to look into your symptoms. Heartburn that doesn’t go away or gets worse could mean stomach or throat cancer. Heartburn can also lead to a condition called Barrett’s oesophagus, which occurs when stomach acid damages the lining of oesophagus. While it is rare, Barrett’s can make you more susceptible to developing throat cancer.

8. Mouth Changes

If you smoke or chew tobacco, you have a higher risk of mouth cancer. Keep an eye out for white, red, gray or yellow patches inside your mouth or on your lips. You could also develop a canker sore that looks like an ulcer with a crater in it. Talk to your doctor or dentist about tests and treatments.

9. Weight Loss without Trying

Pants fitting a little looser? If you haven’t changed your diet or exercise habits, it could mean that stress or a thyroid problem is taking a toll. But losing 10 pounds or more without trying isn’t normal. Although most unintended weight loss is not cancer, it’s one of the signs of cancer of the pancreas, stomach, or lungs. Your doctor can find out more with blood tests and tools that make detailed pictures of the inside of your body, like a CT or PET scan.

10. Fever

A fever is usually not a bad thing — it means your body is fighting an infection. But one that won’t go away and doesn’t have an explanation could signal leukaemia or another blood cancer. Your doctor should take your medical history and give you a physical exam to check on the cause.

11. Fatigue

Many types of cancer cause a bone-deep tiredness that never gets better, no matter how much rest you get. It’s different from the exhaustion you feel after a hectic week or a lot of activity. If fatigue is affecting your daily life, talk to your doctor. They can help you find the cause and let you know if there are ways to treat it.

12. Cough

In non-smokers, a nagging cough is usually not cancer. Most go away after 3 to 4 weeks. If yours doesn’t, and you’re short of breath or cough up blood, don’t delay a visit to your doctor, especially if you smoke. A cough is the most common sign of lung cancer. Your doctor can test mucus from your lungs to see if you have an infection. They may also give you a chest X-ray to check for another problem.

13. Pain

Cancer doesn’t cause most aches and pains, but if you’re hurting for more than a month, don’t just grin and bear it. Ongoing pain can be a signal of many types of cancer, including bone or brain cancer, especially those that have spread, Lepor says.

14. Belly Pain and Depression

It’s rare, but depression along with stomach pain can be a sign of cancer of the pancreas. Should you worry? Not unless this cancer runs in your family, Meyers says. Then you need to see your doctor.

Steps proposed to be initiated:Though as per ICMR Reports, Instances of All Cancer Cases are higher in Men than Women, if data is to be believed, Indian women are more prone to cancer as compared to men. Among females, gynaecologic cancers, including breast cancer (51 per cent), accounted for over half of all cancers.Steps proposed to be initiated as a part of Women cancer awareness campaign

  • Recommendations for breast cancer:

Women older than 40 should have a mammogram every year. Women older than 55 can switch to a mammogram every two years or continue doing it yearly. Screening should be continued until the woman is in good health and is expected to live for another 10 years. Some women are considered at a high risk of breast cancer and should be screened with a breast MRI and mammogram yearly. MRI should be used in addition to, not instead of, a mammogram. This includes women who: ● Have themselves or have a 1st-degree relative been diagnosed with genetic cancer syndromes, like BRCA1 mutation, BRCA2 mutation, Li-Fraumeni syndrome, and others.

  • Recommendations for cervical cancer.

Women older than 25 should have a cervical cytology test (commonly known as a Pap smear test) every 3 years. Alternatively, women older than 25 can undergo testing for high-risk human papillomavirus (hrHPV) every 5 years. Co-testing with cervical cytology and hrHPV test can also be done every 5 years. Screening should be continued till the age of 65 if the results of tests have been normal for the last 10 years. Cervical cancer is unique among cancers as most cases are caused by a virus known as the Human Papilloma Virus (HPV), and we have a vaccine for it. The Indian Academy of Paediatrics (IAP) recommends vaccinating all children, male and female, for HPV between the ages of 9-14 with two doses of the HPV vaccine. Children aged 15 and above can still be vaccinated for HPV but require three doses.

  • Recommendations for colon cancer.

Patients have different options for screening tests for colon cancer. For average-risk individuals, people older than 45 should undergo screening tests. Colon cancer screening can be done by either a colonoscopy every 10 years, a flexible sigmoidoscopy every 5 years, or faecal occult blood tests every year. More frequent screening, every 3-5 years, may be required if colonoscopy reveals the presence of polyps. Screening should be continued until the age of 85. Some individuals are considered at a higher risk for colon cancers and have different screening recommendations. Individuals with one or more family members who have had colon cancer need to be screened with a colonoscopy starting at age 40 or 10 years before their relative got diagnosed with colon cancer, whichever is earlier. They also need more frequent tests. No other types of tests are recommended for such individuals.

  • Public awareness and Cancer Cure Research : Ray of Hope

By knowing and understanding the basics about cancer and following the recommended guidelines for screening, men and women can help reduce the number of cases and increase survival rates by allowing treatment to begin earlier.Unfortunately as per medical records cancer of pancreas and ovaries are detected late. Similarly, more effective Cancer cure research is also needed. Recently, Small trial sees potential for new rectal cancer drug; After receiving an experimental drug called “dostarlimab” in a small trial, all 12 participants experienced complete remission. Doctors say this is the first time that such a cancer drug has had a 100% success rate in a clinical trial. Results of the small study, done by researchers at the Memorial Sloan Kettering Cancer Centre in New York, but it’s too early to call it cancer cure. Newer treatments such as monoclonal antibodies have greatly improved survival.

Over 200 people including doctors, medical staff and local cyclist associations participate in the event to create awareness about Sarcoma Cancer and life beyond cancer treatment

This Sarcoma Awareness Month, Apollo Cancer Centres, India’s best cancer hospitals, hosted ‘Winners on Wheels’ cyclothon to spread awareness about Sarcoma cancer. This initiative was aimed to create awareness and encourage positivity about cancer treatment for a better quality of life. To shed light on how a well-treated cancer patient can lead a successful life, people volunteered to participate in the cyclothon as Apollo’s brand ambassador. Regional Cyclothons across 5 cities – Bangalore, Hyderabad, Mumbai, and Chennai are also being conducted to create awareness about Sarcoma Cancer & its symptoms.

The event was flagged off by Mr. P Shivakumar, Managing Director – Indraprastha Apollo Hospitals and Dr. Shanthi Bansal, Director Medical Services, Indraprastha Apollo Hospitals. The clinicians, doctors and medical staff were present for the event. Local cyclist associations also participated in the cyclothon along with Sarcoma cancer winners.

Sarcoma is one of the cancers which affects the soft tissues or bone. Bone Sarcomas constitute as the third most common cause of mortality in adolescents. In addition to raising awareness around Sarcoma cancer, the cyclothon also promoted the need for a healthy lifestyle to ensure a future with less cancer risk. The gathering discussed the primary topic of reasoning on the forgotten disease.

Commenting on the initiative, Mr. P Shivakumar, Managing Director, Indraprastha Apollo Hospitals, New Delhi said, “At Apollo our endeavor has been to facilitate timely diagnosis and try and curb complexities. However, in case of cancer, many a time it goes undetected, and patients only consult experts when the condition has advanced to a complex stage. Hence the emphasis is always on early detection by identifying symptoms and prevention by seeking immediate expert intervention.”

On the occasion, Dr Yatinder Kharbanda, Senior Consultant, Orthopedic surgery, Indraprastha Apollo Hospitals, New Delhi said, “Sarcoma is a type of cancer that originates from the bones or the soft tissues of the body like cartilage, fat, muscle, blood vessels, fibrous tissue, or other connective or supportive tissue. There are over 70 kinds of sarcoma based on where cancer crops up. The actual cause of sarcoma is unknown, but generally cancer forms due to mutations in the DNA within the cells, but risk factors include inherited syndromes passed down from parents to children, and exposure to chemicals or viruses. However, if diagnosed on time, sarcoma is preventable, similar to this case, where our patient was detected in the early stages of sarcoma cancer, such that with a primary surgical intervention, we were able to treat the patient successfully.”

Sharing his perspective, the patient, said “I would like to thank and acknowledge the team of Indraprastha Apollo Hospitals for helping me feel healthy again. We had lost all hope when I was detected with cancer, but it was because of the support and efforts of the team here that I was able to stay resilient through this journey of recovery.”

The Gamaleya National Research Center of Epidemiology and Microbiology (Gamaleya Center) today announced that Vaccines, the leading peer-reviewed medical journal, has published the results of a joint study conducted by team of scientists including representatives of Moscow’s City Clinical Hospital No. 67 named after L.A. Vorokhobov and the Gamaleya Center showing the two-dose Sputnik V vaccine is 97% effective against hospitalisation caused by the Omicron variant of coronavirus (B.1.1.529) among those vaccinated with 3 or 4 components (re-vaccination with Sputnik Light or Sputnik V after Sputnik V).The corresponding article has been peer-reviewed and is available at:

https://www.mdpi.com/2076-393X/10/6/938/htm

The study was conducted from 11 January to 21 February 2022 involving over 1,000 patients in Moscow. Sputnik V showed high efficacy against hospitalisation at 85.9% among patients vaccinated with at least one component. Efficacy among those vaccinated with 3 or 4 components (re-vaccination with Sputnik Light or Sputnik V after Sputnik V) rose to:

  • 97% against any cases of hospitalisation;
  • 97.7% against moderate-severe and more severe cases;
  • 98.6% against severe and critical cases;
  • 99.4% against critical cases.

As authors note, “the study shows that vaccination with Sputnik V and Sputnik Light has a high effectiveness for protection against hospitalisation. The reduction in the severity of COVID-19 regarding the Omicron variant was also observed. The greatest effectiveness was evident in protection against a critical course of the disease requiring patients to be admitted to the intensive care unit.”

To date, Sputnik V has been authorized in 71 countries with a total population of over 4 billion people, and Sputnik Light has been approved in more than 30 countries. The Russian Direct Investment Fund invested in development and production of both Sputnik V and Sputnik Light.

There are many reasons for it. Firstly, there has been an overall increase in all cancers including the pediatric cancers owing to the fast industrialization and economic growth of the country. Few western countries were in the same stage 2-3 decades back. Secondly, the awareness about pediatric cancers has increased both amongst the parents and the health care providers which has led to increased and early detection of these cancers. Last but not the least, the accessibility of medical services trained pediatric oncologists, dedicated pediatric oncology setups has increased which lead to improved diagnosis and survival or pediatric malignancies. With respect to diagnosis, the diagnostic modalities have widened and their precision has improved significantly. There are significant advances in treatment modalities as well, earlier the treatment of cancer was done with conventional chemotherapy, but nowadays we have targeted therapy and immuno therapy for a number of cancers which greatly improves the efficacy of the treatment. It is also a known fact that the Paediatrics Cancers have much better outcomes as compared to adults if treated in a disciplined manner.”

Taking care of one’s health- both physically and mentally; is also very important and I hope my associating with Apollo helps in conveying this message as well.”
